Form reason fields
You can include a reason field to provide an explanation of the answer or scoring for any question. When the user fills in the form, the reason field appears as a drop-down list of values.
For example, for the question, Did the operator answer the phone promptly?, if the answer is No, a list of reasons for the answer can include the following responses: the operator was not at his desk, the telephone was malfunctioning, and so on.
